Service Description
A structured RN-led recovery support program designed to guide patients and families through the critical transition from hospital to home. This service program includes tailored services across days 1-14 of discharge. Support may include: • discharge instruction review • medication reconciliation • health literacy education • red-flag monitoring guidance • caregiver coaching • care coordination Investment: Starting at $2,000
Cancellation Policy
A non-refundable deposit is required for all services. Cancel or reschedule at least 24 hours in advance to avoid losing your deposit or a package session. No-shows are automatically forfeited. A 10-minute grace period is allowed; after 15 minutes, the visit may be canceled. Packages are non-refundable and must be used within their set timeframes. By booking, you agree to all KLM Cares policies.
